Quiz results exceed 100% correct

Symptoms:
The quiz results for a story can exceed 100% correct if you click Cancel during the Pass step (after evaluating the student's answers to the quiz questions), and the student does not log out and back in. You will be asked to re-evaluate the student's answers to the open-ended questions, but doing so increments the number of questions answered correctly over 100% (for example, 12/9). This problem occurs because canceling does not clear the previous evaluations of the open-ended questions.
Workaround:
You can prevent this problem by logging out (quitting the program), exiting the login screen, and then logging back in as the student. You can then continue with the Pass step. Once the quiz results are over 100%, the only way to fix the problem is to reset the story.
